Mental Health Aftercare and Alumni Support
Finishing treatment is a step, not an ending. Before your last day, your team builds a discharge plan with you, so the next therapist, the next appointment and the next step are already in place.
Discharge planning starts before your last day
Whether you finish in residential, PHP or IOP, your team starts planning what comes next while you are still in treatment. The aim is no gap between Wings and the care that follows.
Step 1
Residential
24-hour care in a home setting
Step 2
PHP
Full treatment days, home at night
Step 3
IOP
A few hours a day, around work or school
You are here
Aftercare
Weekly therapy, medication follow-up, alumni
Early in treatment
Goals for after Wings are part of the treatment plan from the start.
During treatment
Your team drafts the discharge plan with you, and with family if you consent.
Before your last day
The next therapist and prescriber are lined up, and the safety plan is written.
What your discharge plan covers
Every plan is built around the person. These are the parts most plans include.
A therapist to see next
Follow-up therapy lined up before your last day, so there is no gap.
Medication and psychiatry
A medication plan and a psychiatrist or prescriber to continue with.
A safety plan
Warning signs, coping steps and who to call, written down with your team.
Home, work and school
Planning the return to daily life, including work, leave paperwork and housing.
Family
With your consent, family can be part of planning what comes next.
Skills to keep using
The DBT, CBT and ACT skills you practiced, with a plan to keep them going.
Ongoing therapy and alumni connection after the program
The structured program ends. The support does not have to.
Weekly therapy and testingOngoing care after the program
Once you no longer need a structured program, weekly therapy keeps the work going. Wings Counseling Center offers weekly therapy and psychological testing, and is one option your team can suggest. Your discharge plan can also connect you with a therapist closer to home.
AlumniStaying connected
The people you meet in treatment often stay part of your support. Ask your team before you finish about ways to stay connected with Wings and with other alumni.
Family involvement after treatment
Returning to treatment, without starting over
Setbacks happen, and they do not erase the work you have done. If symptoms come back, call admissions. The clinical assessment decides the right level of care, whether that is IOP, PHP or residential.
If you are in danger right now, call 911. For a mental health or suicide crisis, call or text 988.
